Try UV (ultra violet) lamp( "Ultraviolet (UV) Light is a proven technology that has been killing mold, fungus, bacteria, and viruses "), and then clean it with vinegar and alcohol if needed. I use modified air purifier.

Guys, drink the vodka and you soon won't notice any smell!?
Seriously though, simple airing it in a dry secure space will eventually eliminate the smell whatever it is, even if it takes some time,
It took me a couple of years once to get rid of tobacco smoke from an accordion I found in a yard sale. ?

Knobby,
Powdered calcium bicarbonate (baking section of supermarket) is very effective in curing bad breath in a refrigerator.
In this case, one could place the bellows along with a heap of calcium bicarbonate (in an old sardine tin) inside a cardboard carton big enough to hold both and close the lid.
Should be cured in a week!
?

Besides the JimD recommendation, I'd not hesitate placing the removed and extended bellows in a big clear plastic bag and liberally sprinkling some baking soda on them inside and out and leaving them in a bag for a week, then using a vacuum cleaner and new soft painter's brush to clean them off. This is a "dry method" that works well, I saved some pillows for my sister that she left in the shed over the winter, so I know it's safe and does work well with a couple of applications.

This is not a problem I’ve dealt with, but from others I understand that Febreze has proved helpful in many cases.
Fabreeze is a great product, and in some cases leaves a pleasant smell, but I don't really feel it does a good job of actually removing the offending smell as much as covering it, and in compressed spray form, I've seen it really negatively affect some people, especially those with breathing issues. :)

Had the problem once put coffee grounds in the case with the accordion for a short time and it seem to of worked.

I'd be very wary of scattering anything (baking soda, Fabreeze, coffee) about the bellows or reed boxes etc: it may prove difficult to remove again, especially if it got into the reeds !?
Keep whatever you're using together. Heap it in a sardine tin or similar: it will work just as well but without creating additional problems!??

Haven't tried it, but, bag accordion and use an ozone generator to fill bag as many times as necessary. Good stuff for odors.
